Parent or Guardian Declaration and Consent

Yorkshire Outdoor Activity Park takes the health and safety of all participants seriously. Kids Laser Tag is a supervised outdoor activity designed specifically for children. However, like all physical and adventure-based activities, it involves inherent risks.

The activity may involve running, physical movement, teamwork, and play within a woodland environment that may include uneven ground, slippery surfaces, natural obstacles, and fallen branches. Despite all reasonable safety measures being in place, there remains a risk of minor injury.

This declaration is completed by a parent or legal guardian to provide consent for a minor to participate in the Laser Tag activity at Yorkshire Outdoor Activity Park.

I understand that:

• My child will receive a safety briefing and instruction on the rules of the game, the site layout, and the correct use of Laser Tag equipment before play begins.
• Safety rules are in place to protect all participants and must be followed at all times.
• Failure to follow safety instructions or venue rules may result in my child or their group being removed from the activity without entitlement to a refund.
• Protective goggles, where provided, must be worn as instructed and must not be removed during play unless directed by staff.
• Laser Tag equipment must only be used within designated play areas and must not be taken into safe zones.
• Physical contact between participants is not permitted.
• At certain times of the year, insects or other wildlife may be present on site.
• All food consumed on site may contain nuts or other allergens.

I confirm and agree that:

1. I am the parent or legal guardian of the child taking part and have the authority to give consent on their behalf.
2. I give permission for my child to participate in the Kids Laser Tag activity at Yorkshire Outdoor Activity Park.
3. I believe my child is physically fit and mentally able to take part in the activity.
4. My child has no medical condition that would make participation unsafe, or I have informed staff of any relevant medical conditions, including the use of inhalers or other medical aids.
5. I have explained to my child the importance of following safety rules and staff instructions at all times.
6. I understand that Yorkshire Outdoor Activity Park reserves the right to stop my child’s participation if staff believe it is necessary for safety reasons.
7. I understand that Yorkshire Outdoor Activity Park does not accept responsibility for loss of or damage to personal belongings brought on site.

Final Parent or Guardian Declaration

I confirm that I have read and understood this declaration and have had the opportunity to ask questions if required.

I confirm that I will ensure my child listens to and follows the safety briefing provided before the activity begins and complies with all instructions given by staff during the activity.

I confirm that I am legally competent to sign this declaration.
